Can you tell if a guy is a virgin by looking at his penis?

If that were the case every Dr would be able to tell. There is no way of detecting if a male has lost his virginity simply by examining his penis. Unless there is a large message written on it to say he is no longer a virgin. Or unless there is damage to it or if he has had scarring due to an STD, other than that no you cannot tell.
