one of the brain's more interesting functions is being able to create a black-out. If you are in a lot of pain, too much pain, your brain can just lock down. So the worst-case-scenario is the guy will faint from pain.
However, if you're a sado-masochist and experimenting with cutting your balls off, I suggest you don't.
